Default Writeup Sheet Needed

Howdy y'all.

We need a writeup sheet. Things have gone on too long at my restaurant, and after meeting with a labor lawyer, I realize that I need a lot of documentation before I fire someone. I figure, 3 writeups, you get a meeting. Fourth one, you're let go.

Let me know if anyone has one pre-made up, or possibly just the important info.

I'm figuring something like:

______________________________
NAME OF EMPLOYEE:

NAME OF MANAGER:

DATE / TIME :

OFFENSE:________________________
______________________________
__________________________
___________________________

THIS IS OFFENSE #_____

SIGNATURES
___________________________________

Please help me take out the trash.

Thanks

-zak

I'd add two more sections. One for the employee to state their point of view regarding the incident and, more importantly a section where you can lay out expected future behavior and the consequences of further offenses. That last part is what will help you the most when terminated employees try to collect unemployment insurance and you dispute it. It's difficult for a former employee to claim they didn't know the axe was going to fall when you have it in writing.

You pretty much have it, but if you really want to get the nitty gritty, find out your state's laws on unemployment and cause for wrongful termination.

I'm in Michigan and here you need 3 consecutive offenses documentated with witnesses and statements. *As far as I know, within a week.*

So find out the law.
