Hi there , I shall start the ball rolling by naming two of he worst dishes I have ever had the displeasure of having to make .......
Mock pork escalope
Sausage meat ( the cheapest and nastiest) mixed with chopped bell pepper and onion , rolled out really thin , cut out with a baked bean tin and fried
Mmm my mouth is still watering .
Bizarre pizza sauce ......
At a large function ( Party in the Park) , which I truly hated having to work . They had many concession stands ,I was lucky enough to get the pizza tent .
The executive chef had not purchased enough pizza sauce , so we ran out it was substituted with ketchup , apple sauce and curry sauce mixed together
This was the point I walked away in disgust .

Clam Chowder. I seriously hate it. Done right, it is a good thing, but even then, I hate it! I worked at a seasonal resort once. We were so busy, and worked in such a small kitchen that we would take a bunch of canned clams, some pre diced/cooked potatoes, onions, celery, throw it in a big pot of boiling heavy cream, and thicken it with corn starch. We would sell gallon after gallon of that crap and people loved it for some reason. Yuck! And this was in the middle of summer! Who wants to eat clam chowder in the middle of July?

My former kitchen manager ran Spam Empinadas as a special. With cheddar, green olives and RR peppers. He made them out of phyllo and they were all different sizes and shapes.
He also did a chicken and hotdog stir fry with marinara sauce. Facepalm.

Chicken a la King.

I was once a food service supervisor for a private children's psychiatric hospital, and the chef brought out a hotel pan of Chicken a la King, placed it on the line and walked away. The sauce was literally neon lime green. I still haven't been able to figure out how he managed that.

One of the hospital staff employees looked at it with trepidation, but I guess he was hungry enough, so he served himself anyway. The perplexing look on his face was enough for me to walk back to the kitchen and voice my concern to the chef. He promptly took the pan away, and with a dash of Kitchen Bouquet/Grave Master solved the problem. Brought it back to the line and all was well...until the hospital staff employee came back for seconds, took one look at it, then looked at me and asked, "wasn't that green before?".

I swear, I ran into the kitchen and I just about died laughing.
